If you would like to make an appointment, please call our office at 570-322-0609 or 800-692-6226 between 8:30 a.m. and 5:00 p.m. Monday through Thursday, or 8:30 a.m. and 12:30 p.m. on Friday.  Patients are seen by appointment only.

Our physicians accept most major insurance companies (Medicare, Blue Shield, Workers' Compensation).  We participate in most Health Maintenance Organizations (HMOs), Preferred Provider Organizations (PPOs), and Managed Care Systems.  Our staff will gladly file your insurance for you.  It is, however, the responsibility of the patient to know which physicians are on his/her insurance panel.  It is also the patients' responsibility to get the necessary referrals and to bring their insurance cards and information to the appointment.  If you are being seen as a Workers' Comp or an accident patient, you will need to bring your work related injury report or accident insurance information with you. If you do not have the necessary paperwork when you arrive, your appointment may have to be rescheduled.

Patient payment is due at the time services are rendered.  This includes co-pays, deductibles, and any balance not covered by your insurance.  Balances not paid within 90 a day period will be turned over to a collection agency.

When coming to your appointment, our patients are asked to please bring their correct insurance information, medicine list, x-rays or reports relating the the visit, any referral if needed, and work related papers if needed.
